§ 13-64. Forms.

  • The city administrator shall cause to be prepared and made available at the city collections department the forms necessary for compliance with the registration requirements of section 13-61 above. Initial registration and any renewal registrations where there have been changes in ownership, location, or nature of business shall not be approved until such time as the applicant has provided satisfactory proof, as determined by the head of the collections department, that all sites of operation of such applicant's trade, business, vocation, or calling, have met all applicable codes of the city and state, including, but not limited to, sale of alcohol, fire, building, and health, and any other codes that may be in effect and designated as a requirement by the city administrator.
