§ 26-63. Application for permit.

(a)

An application for a permit required by section 26-62 shall be submitted in writing to the police department of the city, on forms provided by the police department for that purpose, for a taxicab driver's certificate.

(b)

Such application shall contain the applicant's name, age, and address, and the applicant shall furnish to the police department his fingerprints, one (1) print of a photograph of the applicant and proof of a current, chauffeur's license issued by the state.

(c)

The police department shall cause a criminal history inquiry to be conducted on each driver applicant, the cost of which, if any, shall be borne by the driver applicant.
