Application for participation in the program shall be made to the Sebastian Retired
Citizens Association, Inc. (SRCA) at locations to be approved in advance by the city
administrator. Eligible applicants must re-certify their income each year. Income
verification shall be based on the applicant's income tax return for the previous
year. If no such income tax return has been filed, the applicant shall furnish his/her
year-end social security benefit statement and proof of all other sources of income
for all residents of the household. The SRCA is authorized to make a determination
of eligibility of applicants, and may use such available sources such as governmental
agencies to verify the information contained in the application form. The forms for
application shall be approved in advance by the city administrator. The city administrator,
or his designated agent, shall confirm or reject all such determinations of eligibility
for the program.
